I've heard of a few. It can grow back in rare cases.
DH 's surgeon said he'd only had one that failed out of hundreds & that was because he didn't follow post-op instructions. It's also caused from men who don't get the follow up test.

Did he go back for the test to ensure it worked properly? My hubby never did and this still concern s me!

A friend did but that’s because he didn’t follow the so many ejaculations or so much time after the op rule. So basically he impregnated his wife with her 4th baby with some straggler sperm.
